BACKGROUND CHECK CONSENT

I hereby authorize the Noxubee Titans organization to conduct a background check as part of the volunteer screening process. I understand that this background check may include but is not limited to verification of criminal records and professional references.

I consent to the release of all information requested by the Noxubee Titans organization to conduct this background check. I understand that the results of this background check will be treated as confidential and will only be used for volunteer-related purposes.

I also acknowledge that any misrepresentation or omission of information on my part may result in the withdrawal of any offer of volunteer services or termination of volunteer services if already approved by the Noxubee Titans organization.

By accepting the terms & conditions below, I confirm that I have read and understand the above information and voluntarily consent to the background check process.

WAIVER OF LIABILITY

I hereby acknowledge and agree to the following terms and conditions for participation in the Noxubee Titans Sports League: I understand the risks associated with sports activities and release the Noxubee Titans Organization from liability for any injuries or damages. I consent to the use of photographs or footage for marketing purposes and acknowledge that equipment provided must be returned after the season. Failure to return the equipment may result in exclusion from the awards ceremony and forfeit the opportunity for future volunteer services with the Noxubee Titans Organization. I understand that my name and image may be used in promotional material without further consent. I also acknowledge that by volunteering with this organization, my social media and public image will be a representation of the Jeffery Simmons & Noxubee Titans brand and that any posts or actions deemed inappropriate will forfeit my opportunity to serve as a volunteer. By signing this form, I waive certain legal rights, including the right to sue.
